OSHA Fines Cleveland Manufacturer $89k for Unsafe Conditions

A Cleveland, OH manufacturer of aerodynamic and emission control products was fined $89,000 by the US Department of Labor’s Occupational Health and Safety Administration (OSHA) on June 10 after inspections in January and April revealed 17 safety violations.

OSHA’s investigation of Soundwich Inc.’s facility began in January, when a 55-year-old worker severely injured a finger in the moving parts of a coil tilter. The agency found that the equipment lacked proper safety guards that would have prevented a steel coil rolling to the side off of a radius pad and injuring the man’s hand.

“Each year hundreds of workers suffer preventable and life altering amputation injuries because employers like Soundwich fail to install required safety guards and mandate their use,” OSHA’s area director in Cleveland, Howard Eberts, said in a press release.

Another investigation was opened by OSHA in April after a complaint said that working conditions were unsafe.

The agency’s Cleveland office said the company:

  • Failed to install machine guarding on a spot welder, drill press, stamping press, and other operating machines.

  • Exposed workers to fall hazards due to a lack of a properly designed guard system.

  • Did not establish a permit confined space program.

  • Failed to locate, mount, and inspect portable fire extinguishers.

  • Did not ensure compressed air for cleaning be reduced to 30 psi.

  • Exposed workers to numerous electrical safety hazards, such as junction boxes without cover plates.

  • Failed to keep floors clean of oil and other materials creating slippery working surfaces.

The citations were announced by the agency on June 13. The company was issued with one repeat violation, 15 serious violations, and one other-than-serious violation.
