A worker was killed after becoming trapped in a conveyor belt at the Orgain Ready Mix Concrete plant in Murfreesboro, TN Nashville, TN Fox News affiliate WZTV reported Friday.

The man was “breaking ice off of a conveyer (sic) belt when he became caught in the machine,” according to an accidental death report filed by the Murfreesboro Police that was included in WZTV’s coverage.

A man and a supervisor were “unsuccessful” in efforts to free the man from the conveyor system, the television news station reported.

The worker’s age is unclear, with the WZTV report listing his age as both 61-years-old and 51-years-old.

The Tennessee Occupational Safety and Health Administration (TOSHA) told WZTV that the death was accidental and that two safety officials responded to the incident to investigate.
