An Alabama-based tank cleaning firm was issued a $171,281 fine by the U.S. Department of Labor’s Occupational Safety and Health Administration (OSHA) after investigators identified a number of safety violations. 

The agency cited American Remediation and Environmental Inc. of Chunchula, AL with confined space, explosion, and fire hazards. 

An inspection revealed that the company permitted entry to a tank without performing testing for atmospheric hazards; neglected to provide personal protective clothing; allowed the use of a non-explosion proof vacuum to clean a tank used to transport a hazardous chemical; did not use a retrieval system when workers entered a permit-required confined space; and did not properly maintain testing and monitoring equipment for confined spaces. 

American Remediation and Environmental has 15 business days after receiving the citations to comply, challenge the findings before an independent board, or request an informal conference with an OSHA area director.
