A 60-year-old worker was killed and another employee was injured as the result of an incident at the KapStone Paper and Packaging Corporation paper mill in South Carolina on Sunday night.

“An incident occurred at our North Charleston, SC paper mill involving employees performing maintenance work in an operating area that was offline for repairs. The incident resulted in the fatality of one employee and injuries to a second employee,” a May 1 press release issued by KapStone said, noting that the injured worker was taken to an area hospital and later released.

The incident occurred at 8:45 p.m. Sunday, the company said. Local CBS News affiliate WCSC reported that Charleston County Dispatchers said a person was entrapped during the ordeal.

The South Carolina Department of Labor, Licencing, and Regulation is investigating whether OSHA rules were violated and if lapses in regulatory adherence led to the accident, a representative of the state agency told WCSC. OSHA is also investigating the incident, according to KapStone’s press release.

Both the company and officials have not identified a cause of the incident.

Employing some 6000 workers across its sites, KapStone produces unbleached Kraft paper and corrugated packaging products, the company’s website said.
