A worker was fatally injured during a coal pile collapse on November 5 at a Cemex cement plant in Knoxville, TN.

Plant staff found the man’s body under the coal and called first responders to the Cement Plant Road facility at about 7:45 a.m., news broadcaster WBIR reported. Crews worked for several hours to remove the remains from the pile.

Knoxville Police identified the worker as Thomas Mitchell, 21, of Blaine, TN, according to ABC News station WATE.

Cemex confirmed the worker’s death to CBS News affiliate WVLT. The company is cooperating with officials as an investigation into the incident is conducted.

“Our condolences go out to his family and all who are impacted by this tragic loss,” A Cemex spokesperson said to the station.
